CMNtv Membership Forms
Download and fill out these forms to join CMNtv.

	Membership Log-On Form Must complete this form to participate in production, playback or for Media Board use. Resubmit annually w/membership fee. Complete and return to CMNtv with Statement of Compliance form (below). Annual membership fee dependent on Resident/Non-Resident status.
	Statement of Compliance Must complete this form and Membership Log-On (above) to participate in production, playback or for Media Board use. Resubmit annually w/membership fee.

Production Forms
Forms for Production and Program Submission/Playback.

	Playback Request Form Must submit with any program intended for playback on CMNtv.
	Media Board Submission Form Template for submission of announcements seen on CMNtv's Media Board. Membership required.
	Underwriting Agreement This form needs to be completed and submitted with any program that contains underwriter support recognition.
	Tricaster Check-Out Form This form indicates a producer's desire to utilize the TriCaster Mobile Studio. It requires information related to the location, date, time of shoot, contact parties, etc. Must be submitted to CMNtv at least 2 weeks prior to the requested date.
	Production Van Application Form This form indicates a producer's desire to utilize the Production Van. It requires information related to the location, date, time of shoot, contact parties, etc. Must be submitted to the CMNtv Production Van coordinator at least 2 weeks prior to the requested date.
	Production Critique Form This form may be submitted by a producer for CMNtv to evaluate and offer advice on how to improve a program they have produced.

Personnel Forms
Forms for Crew Members, Talent and related entities.

	Individual Talent Release This form, when completed and signed, provides documentation that on-camera talent has given permission to appear on a television production destined for playback on CMNtv.
	Group Release Form This form, when completed and signed, provides documentation that on-camera talent has given permission to appear on a television production destined for playback on CMNtv. The form is concise and suitable for large groups as it accommodates more names per page.
	Parent/Guardian Permission Form This form must be completed by parents or guardians who enroll a minor (age 16+) in CMN's Production Certification Class Series.
	Location Release Form This form, when completed and signed, provides documentation that the owner or management of a particular location has given permission to record a television production for playback on CMNtv.
	Crew Call Form This form is helpful to producers that wish to organize their crew positions prior to a shoot.
